Bivalirudin is a synthetic 20-amino acid peptide and a direct thrombin inhibitor used as an anticoagulant in percutaneous coronary intervention (PCI) and for the treatment of heparin-induced thrombocytopenia (HIT). It consists of a hirudin-like N-terminal domain that binds to thrombin’s active site and a C-terminal domain that interacts with the fibrinogen recognition exosite, providing rapid and predictable anticoagulation with a short half-life.
